                                     INFORMATION, PROGRAM AND REGISTRATION

For multimorbid patients, the coexistence of multiple chronic conditions necessitates a fundamental shift in the care model: treating individual diseases is no longer sufficient; instead, the focus must shift to the person as a whole—encompassing biological, psychological, social, and environmental dimensions.

Furthermore, the use of multiple, varied medications (polypharmacy)—often resulting from prescriptions by different specialists—carries a high risk of dangerous interactions, adverse events, reduced treatment adherence, and inappropriate prescribing.

Healthcare professionals therefore face the daily challenge of balancing treatments, benefits, risks, costs, and clinical priorities within an increasingly complex care environment.

This situation makes it essential to develop an integrated, multidisciplinary approach based on a collaborative network involving cardiologists, diabetologists, pulmonologists, nephrologists, general practitioners, and other specialists, in order to ensure effective, personalized management for patients with comorbidities.

Multidisciplinary and multiprofessional management of the chronically ill patient enables comprehensive care from the post-acute phase through to long-term chronic management; it improves adherence and treatment response, facilitates timely access to appropriate therapies, and allows for the effective management of disease flare-ups.

It is within this context that the "One Health" paradigm applies, highlighting the interconnectedness of human, animal, and environmental health.

For patients with chronic conditions, food quality, dietary patterns, food safety, and environmental impact directly influence disease progression, inflammatory status, the microbiota, frailty, and quality of life. It is therefore essential for physicians and all involved healthcare professionals to redefine their roles and professions within a renewed system—one that moves beyond outdated, fragmented, and siloed approaches to patient care in favor of a team-based model.

The course focuses on chronic and post-acute conditions where professional roles overlap and integrate, necessitating the development of a truly "holistic" understanding of the individual patient.

The course aims to provide the conceptual and practical tools needed to address clinical issues based on scientific evidence and current best practices, integrate them into a multidisciplinary approach, and translate them into shared, person-centered care pathways.

Engaging with complex issues enables the adoption of a management strategy for chronic patients that cannot be reduced to a single disease or fragmented into disjointed interventions; instead, care must be built around the whole person, embracing a comprehensive view of health.
